A: Employees may be eligible for FMLA leave if they meet the following criteria:
- Have worked for the employer for at least 12 months.
- Have completed at least 1,250 hours of service during the 12 months immediately preceding the leave.
Q: What are the circumstances in which an employee can request FMLA?
A: FMLA can be requested for the following reasons:
- The birth or placement of a son or daughter for adoption or foster care.
- To care for a spouse, son, daughter, or parent with a serious health condition.
- For a serious health condition that makes the employee unable to perform essential job functions.
- Any qualifying exigency related to a spouse, son, daughter, or parent being on covered active military duty.

A: If you meet the above eligibility requirements, please follow these steps:
- Log into ADP and navigate to: Myself > Employment > Total Absence Management. Select “Request Leave.” This step is mandatory for your leave to be considered.
- Have your healthcare provider complete the attached FMLA Certification. Submission of this form is required for your leave request to be reviewed and approved. Both Certifications are attached to this FAQ for download.
Q: Is FMLA a paid leave of absence?
A: No, FMLA is not a paid leave of absence.
Q: What will happen to my PTO and Floating Holidays I have available?
A: If you are placed on a leave of absence, any available PTO and Floating Holiday hours will be paid out via direct deposit on the pay cycle when your leave begins.
Q: I am enrolled in Benefits through Liviniti, how do I pay my benefit premiums while on leave?
A: You may pay your benefit premiums via check during your leave or establish a payment plan upon your return. Please submit an HR Ticket so you can confirm your benefit premiums before you start your leave of absence.
